OS&D notes clerks will actually finish
The clerk at the door has a scanner, a driver who wants to leave, and a supervisor on the radio. A narrative box will be skipped. A line for piece count, a line for seal number, and a line for dock door will usually be filled.
Photographs belong with the note the same hour, not “later from the office.” Later never comes. If your policy requires a photo of the seal and of the damaged carton, say so on the slip in six words, not in a binder on a shelf.
Reconsignment is not OS&D. Mixing them on one pad makes shortage claims and address changes share a queue. Give reconsignment its own clip. The exception desk review almost always finds those two pads have been used interchangeably for years.
Finish means a closed time and a named owner. An OS&D note that stays “pending claims” without a person is not a record; it is a rumor. The local tracking copy on a desktop machine can store the closed time. It still cannot file the claim for you.
Workshops spend the last half hour filling a sample note from a staged pallet. People remember the motion more than the lecture.
